CONSULTATIONS 

There is no charge to come in and meet us and discuss your health concerns and see if our approach may be beneficial for you.  These are generally 15 minutes long and can be scheduled by calling our office at (773) 878-7330.

INITIAL EXAM 

The fee for the initial exam which includes a physical exam and health history is $78.50 which is due at the time the appointment is scheduled and is non-refundable if the appointment is cancelled without 72 hours notice. 

TESTING FEES 

As a policy, Essential Health Solutions does not quote prices for testing until after you have had an exam.  Since everyone's health condition and goals are unique, it is impossible for us to tell you what testing the doctor will recommend.  Lab testing also involves many unknowns such as reflexive and add-on testing that make it difficult for us to quote prices before you have had an exam.  After your exam, once the doctor has made his recommendations for testing if needed, prices and options can be more accurately provided and explained to you.

INDIVIDUAL NUTRITION COUNSELING

REVIEW OF FINDINGS & TEST RESULTS / INITIAL NUTRITION AND LIFESTYLE RECOMMENDATIONS

There is no additional charge to review your test results with you and for us to review with you the menu plan, recipes, grocery list, pantry conversions, brands we use, and nutrition and lifestyle recommendations that we provide you on your 1st visit after your exam.

PHONE CONSULTATIONS

There is no charge for phone consultations to discuss testing results or dietary recommendations that we have provided to you. 

ADDITIONAL INDIVIDUAL NUTRITION AND LIFESTYLE CONSULTATIONS

Everyone needs a little extra help implementing changes every once in a while.  Additional consultations on Nutrition or Lifestyle recommendations are $109.80 per hour.

INSURANCE 

We are an out-of-network provider for all insurance companies.  We have chosen to do this to keep testing costs down for all of our patients.  If you have health insurance and would like to file a claim with your insurance company, we will provide you with everything that you need to do this, including an itemized receipt, diagnosis codes, and CPT codes.
